The way I figured it was she hung her brother while her sister watched. Sent to the loony bin and eventually released to live with her sister who, traumatised by the event, cross dresses as the brother. Eventually snaps again and bludgeons her sister to death. Goes crazy, starts to hallucinate, and eventually either hangs herself or is sent back to the loony bin with some dolls she thinks are her siblings.

Been awhile since I've played it, so I could be a bit off.
