Note that the hack is still an ongoing project at this point, and there have been several extra patches since the first .ips was released, to fix some problems, correct touhoumon types/moves, correct sprite inconsistencies, etc. Unofficial patches for new/extra areas have also cropped up, though they're not as stable.

The original 151 have all been replaced in one way or another, along with a few extras and later-introduced evolutions of the original 150. Pokemon types, base stats, wild held items, movelists, and TM/HM compatabilities have been changed, and a few moves and abilities have been reworked or renamed for the Touhou theme. Encounter locations and conditions have also been changed (don't expect the same amount of variety in the same patch of grass as in FR).

What hasn't been changed is (for the most part?) the in-game dialog and probably most trainer lineups (aside from direct replacement of what they had into touhoumon versions, of course).

Basically, the pokemon are redesigned within the existing gameplay system and plotline (or lack thereof).

If you're using the semi-translated patch, then probably, otherwise, I don't know.

If it's first-evolution basic forms, then it's probably chibi, if it's rare parody versions then it's probably a reference to something else; I know there's Advent Cirno versions, and after glancing at the lineup in http://www.youtube.com/watch?v=WvsQuXlbhwI and http://www.youtube.com/watch?v=cyhs580fB-s there seems to be at least one other set of un-evolving parody versions; the Advent Cirno ones are prefixed with an A, and other letters seem to be a theme for other parodies. I think the patch shorthand is more likely, though.

Reisen is only in Mount Moon, so you missed that, but Patchy is right up in the grass north of Cerulean. She teleports automatically like Abra, so be quick.
