You have to understand that people back then not only have to pay full price for it, on the top of probably having to delay getting Rance 6 and the hype from such a well-known series from a well-known company, and the view on childhood friend characters post ToHeart1. The "NTR" is just off-screen bullshit with the boyfriend groping Tamaki at the absolute worst and that scene only shows if you're playing like a gigolo (with the mandatory "NTR" scene is just MC caught Tamaki and her boyfriend getting out from a love hotel and the first sex scene having her telling MC that she's no longer a virgin), but the fact that the typical dating sim buyers do not expect it at the time on the top of all the hypes makes the cut run deep into the buyer's heart.

I don't know if elf was still having their old staffs back in 2004 before 下級生２ development, but I think if the competent ones are still there, they would be more aware of the current market and not creating Tamaki in the current state as main heroine. If Tamaki hadn't happened, elf would still probably alive, but still a shell of their former self and bound to producing normal eroges. I don't know if a true remake of the original series would be popular in the current market where heroines are virgins and the gameplay in eroge are mostly little to non-existant except for a few titles.

It's pretty fun to check 2ch archive for the release aftermath reaction of the game. The amount of salt there is astounding.
