Because Lion isn't real and Lion's world isn't real either. It's a world imagined by Yasu. A lot of things don't add up in that world, including Lion's appearance and ideas about gender. He's a projection of a perfect self imagined by Yasu, who can't deal with the fact that the "real" Lion would just be a normal masculine dude with brown hair.

This confirms what I'm saying. Shannon is the main "personality" because she was raised as a girl her whole life. She is a girl. Kanon only appeared when she realized that she's not really a girl at all but a castrated male eunuch without a future.

And they aren't even personalities, as I said. They are different sides of a single person, characterized as tulpas in her head.
