we have two factors here to look at: the fogginess of her mind and the "wakaranai" part
the former can mean that her mind is like that either from the blowjob or from the immorality of the act (this could be perhaps solved if the meaning of the previous sentence was ascertained)
the latter can mean that she either really doesn't understand what he's saying (he's probably suggesting what sexual stuff they're gonna do next) or that she doesn't know herself what they're going to do next (the difference being that the former interpretation implies he's the initiator of the act and the latter that he asked her what to do next i.e. he's telling her to say what she wants to do next)
