Quite a few AV actresses died of HIV, but being Japanese they always keep quiet about their condition, so you never hear about them being infected, and their death doesn't even reach the news unless they've kept their superstar status after retirement.
Both Ai Iijima (probably the most successful AV idol of all times) and Hotaru Akane (huge name until her retirement 10 years ago) pretty obviously died because of AIDS.
Some anon mention a Tabuchi interview where he talked about the rampant issue of STDs in the 90s, but I haven't read it.
