I was thinking about how sandworms breathe the other day. Obviously, lungs in the human part of her body would not be capable of supplying enough oxygen to her entire body. In fact I believe that her human body doesn't contain any vital organs, only her reproductive system. But back to the point, I was thinking that, sort of how earthworms breathe, they absorb oxygen through their wet skin. If worms dry out, they suffocate and die. Sandworms however don't have a wet skin on the outside; it's a tough chitin, but it's plated. So between the plates of a sandworms chitin, underneath it, there's wet, mucous-like skin that allows for the absorption of oxygen through it. Not only does her tough chitin protect her body from damage, but it also protects this soft skin from drying out in the arid desert heat.

This is just my headcanon, but I've always had the idea that being inside a sandworm's mouth is the highest honor with her, that it's just as intimate as sex itself. If someone was to try to force themselves inside, it'd be equivalent to rape. Only her husband is allowed inside, ever. The only exception to that is her newborn babies, and that's just for a short period of time until she's big enough to swim the sands herself.
