Consider this, despite the presence of hardcore porn in the west, softcores and sexploitation movies are still a thing there. That means the demand is still big enough. There are still many people that can easily get off to simulated sex.
Instead of looking at censored JAV as downgraded hardcore porn, you should look it as upgraded softcore instead. You don't get to see the genitals (clearly) but at least the mosaic is thin enough that you can see that the sex is real and not simulated unlike softcores.

Oh and about the teeth, it could be vary between people. There are those that has naturally white color like you, and then at one point the people with natural yellowish teeth think they look cool and start the trend of teeth whitening.
