I don't doubt that Erika is not a full-on body double. One look at her face makes the difference apparent. I think she was just granted aspects of Bern's appearance as a symbol of servitude. Not unlike how Lambda gave Beato blonde hair, funnily enough But even then, having someone who shares traits of hers being called into question would still set Bern off.

It depends on the artist. Bern at her biggest is bigger than Erika at hers. But Erika is more consistently portrayed with actual breasts than Bern is, meaning that more often than not she's bigger. Still, it really comes down to which interpretation you choose.
