Keiichi became paranoid because the town was creepy and he convinced himself that whoever killed Tomitake was after him too for being an outsider and sent Rena and Mion to erase him. It wasn't paranoia for paranoia's sake, that's flanderizing the syndrome and treating it like a cold plot device that affects everyone the exact same way regardless of the circumstances. Keiichi had every reason to be afraid in this new community with all those weird rituals and shit. Again, Irie himself explains that not everyone listens to the paranoid thoughts unless it's they have sufficient reason to believe them. This isn't the case with Rena, who was simply at school with her acquaintances.

Rena's syndrome was triggered when her mother ruined the family. She hated herself for not doing something sooner and started seeing maggots in her blood, because she convinced herself that her life went to shit the moment they left Hinamizawa. She convinced herself that she was cursed by a supernatural entity and her life would get worse unless she returned to the village. Rena had no reason to believe the paranoid thoughts, she brushed them off easily. But she associated the itches with maggots in her blood and came up with the curse explanation, that's the symptom that made her insane. The itchiness. While in this state, her supposed friends tried to do something sexual to her and she finally snapped, hitting the guy and going throughout the school breaking windows out of rage and partially insane. As I explained, this has nothing to do with any baseless paranoia.

Satoko does react to the paranoid thoughts but, as Irie explains, she's good at ignoring them while they accumulate until they explode suddenly. She doesn't slowly lose her mind like Keiichi. She acts normal but her brain is already conditioned to believe insane suspicions at the drop of a hat if something triggers her fears. And yes, unlike Rena, she does have reasons to listen to the paranoid thoughts and lose herself in that madness. She could suspect her uncle is coming back and convince herself of it because she saw a random motorcycle heading to town. That kind of stuff.

Shion also listens to the paranoid thoughts, but it's more that she convinces herself that the Sonozakis are murdering dissenting villagers and believes every single one of her own suspicions.

Rena reacts to both the itchiness AND the paranoid thougts in Tsumihoroboshi, when she starts believing in the aliens, small inconsistencies make her thing Mion sold her out to the police, etc. But that has nothing to do with her Ibaraki incident, where she was simply a normal student without any reason to fear or be paranoid. She just had massive family problems and believed in a literal demonic curse due to the itchiness and the maggot delusions, which caused her to snap at the middle school molesters in an outrageous and disproportionate way.

Fair enough, but you MUST apply the sound effects patch. Those cicada sounds ruin the atmosphere and sound awful. What were they thinking? Did they expect some random dude to recognize the cicada recordings Ryuukishi used and sue them? Why replace them with such garbage?

Their release had a bug that made this shit loop over and over again in many scenes, even though it's supposed to play only once.
