I've reasoned it that she could have both. Having a permanent eye growth be visible through clothing has got to be a wardrobe nightmare waiting to happen. I always imagined it like a hard jelly thing like ballistic gel. A bit flexible and squishy but still has some firmness to it. I've been in the camp that her third eye was apart of her body and not an accessory, so I cant imagine it would be comfortable to have a large round stone fused to your cleavage… that is if she isn't a washboard.
