I've always wondered if Medicine was Alice's daughter. They look similar and they bother have an association with dolls.

I remember seeing this "essay" someone made about Medicine actually being the corpse of a dead child that was abandoned at nameless hill and is being controlled by the smaller doll.

It could be that Alice was forced to abandon her daughter (be it a painful sickness or just Alice being a horrible mother) with that small doll. Eventually the doll was infected with a soul and began to use the child as a proxy puppet.
