Someone who is in a mental hospital to treat that disorder doesn't really have what Miyao has.

In real life, people who are born with DID don't necessarily have any traumas or troubles associated with it, so they are technically not "disordered". It depends on the brain, but it can often be a benign condition where the personalities work together during their daily life and create rules for when each one is allowed to become dominant, in case they want to and aren't satisfied with just observing and talking to each other.

Generally, more than two personalities active at the same time will strain the brain, causing headaches after a while, so they tend to spend time together in pairs.
