Yeah, Suwako was originally supposed to look like most youkai in terms of age.. that is, somewhere in late teens/early 20s. She's an old hag of old hags, an old god among the gods themselves.

And she doesn't even really look much like a kid in-game. She just looks younger than Kanako and doesn't appear to have a very serious personality, and so people seemed to take it that she was a little kid god who couldn't stand up to a full-grown one. ZUN didn't care enough to correct people on this, and so loli-Suwako stayed. Pic is one of the very very few fanarts I've seen that depicts her like how she actually looks in game.

Still haven't had one since that time I was Suwako in my dream. It was pretty awesome, I didn't give a shit about anything and instigated a fight between Sanae and Kanako which ended with Sanae delivering a flying kick straight into Kanako's face.

After the dream, for a few minutes, I felt that I truly understood her mentality /situation to a depth I can't describe.
