Does Touhou exist in Touhou?

That is, in the Touhou universe, does Touhou exist in the outside world? Was Sanae, being an otaku, a Touhou fan before she came to Gensokyo? Does Yukari tell stories to ZUN, who then adapts them into his games?

Is Touhou getting more or less of a following, these days? The foreign fandom might be hard to track (unless you believe you can get relevant numbers from the traffic of shrinemaiden, pooshlmer, or whatever the main Touhou fanbases outside /jp/ are), but Japanese fandom might be gaugable by number of doujins, game sales (does ZUN ever even print enough copies?), or some other metric.

Touhou fandom growth as a whole: Speeding up, or slowing down?

Compared to, say, a year ago.
