Depends on whether or not they've been taught it's something they shouldn't bee shown, and how old they are. Some cultural differences as well, though I'm not that well traveled.

Before the age of 6 they won't think much of it, unless it's erect and you wave it around so they'll be curious as to why it's different from their father's / brother's / their own thing. Up to the age of 10 they would be slightly embarrassed and curious; they want to look at it since it's different, but they feel they shouldn't stare at it.
After that they usually know it's indecent, and a couple more years later they've probably already sucked a ten or so.
