I think it started with a new take on bullet hell, no one really made bullet hell games based on eye-candy-like intricate patterns of bullets rather than semi-random streams of bullets in the style of Dodonpachi and everything else in the genre. In addition to that, the usage of lolis, however poorly drawn, rather than hulking mecha-ships probably added a lot to it. Not only that, but the lolis had some sort of story element to it, even if the canon stories aren't particularly fantastic they offered more than other games of the genre. There is also the music, the fact that the games were well made even though they were doujin, with most of or all the work done by one man, and several other factors that made Touhou an empire loved by many rather than most obscure doujin games.

tl;dr, Zun really made a very original product, something that brought together many enjoyable elements that people unexpectedly had a demand for.
