It's a couple, there is another instance of the translation where they clearly wanted to avoid him saying "I'm a guy" so they reworked the dialog in a totally different one. This said, the translation is pretty gendered and they exclusively refer to him in male pronouns and even words like son, it's more like the (twitter) fanbase saw that line about binaries and ran with it. Other translations curiously translated both lines fine as well in general being more accurate to JP.

I guess the translator saw that line and went for it but couldn't fundamentally "fix" the fact the there was no gender ambiguity in the original script (The crossdressing was literally just inserted because the JP fanbase liked it, originally wasn't a thing past the prologue).

I also think it's at odd with the story the difficulty in their relationship is that fundamentally Hijiyama comes from 1945 so he can't cope with being attracted and bein gin love with a guy, which wouldn't be a problem if Okino didn't see value in his own sex. There is a scene later in the game where Okino basically confesses to him but Hijiyama's rejects him because "Who would want to be confessed by a guy" and of course the fact their original were show to be two men in love.
