His drawings are rudimentary and the shmups games are not that complex compared to most of the games out there, but then you have
- original music that works and has really catchy melodies
- designs that are still unique despite being basically lolis in frilly dresses
- carefully designed setting that refers to history, classical culture and pop culture
- consistently hard work on expanding the universe
- big luck, no doubt about it

It's not any one thing. It's the characters, and the music, and the gameplay, and the openness to people making money from his work, and th

ZUN's not great at anything (except maybe programming), but he has great ideas, and those ideas are inspiring to others who ARE great at things and want to reinterpret the kernels of what he laid down into what they feel they're meant to be. Millions of people having millions of different interpretations means an endlessly thriving fanbase.

Probably already mentioned, but he's super hands off with the fandom. He's more than ok with people making money off of his work, and supports other people making and selling their own games, manga, and even anime based on the characters, settings, and world.

Japan is usually ok with doujin, up until it gets too big. Even then, the original artists don't usually let derivative stuff go mainstream. Touhou is very different in that regards (despite not being mainstream of course).

You should keep in mind that he did not do those designs alone. He also programmed his own games and he's also a composer with music being, arguably, the primary driving force behind popularity his works.

The man is an actual genius who carved his own path with his own hands (unlike people like, say Kojima, who just orders other talented people around and slaps his own name on top of result) and people who attempt to single out his simplistic designs alone as some form of personal critique are fucking insane.
