This is a bit hard to critique because of the unusual nature. What I can see is that the light seems to be coming from behind him so I added more sharp rim lighting. This is mostly on the head hair ears and shoulders and the balloon

The face could use some more emphasis on the geometry so I added shadow to the edge of the mouth around the eyes and nose.

Also the character and the balloon seems to made of some liquid material so I added a lot of shiny highlight spots to emphasize this effect. Normally this doesn't go for skin but this is surreal here.

The hand anatomy needs some work I know this is surreal but it is always good to know how to do proper hands, it can add so much to the creepiness.
