You absolutely do not have to get hard, if anything that would make it more difficult with the pressure of the bloodflow forcing the ink out. It;s like an eyelid tattoo, you stretch the skin over something cylindrical like a (wrapped up) waterbottle. Probably the one time I can actively get involved and help an artist out.

It's easy enough to do. What our "clever" friend doesn't realise that day to day absolutely nobody sees my work, and would never in a million years guess I was absolutely covered.
It's quite enjoyable, I hope to one day see a tirade of attacks against something a person dislikes (dressed up as something academic) in person without them having any notion that I'm one of those very things.

That is broadly how it works, yes. You can provide references and have them draw something up, pick something from their flash (pre-drawn work some artists post) or find an artist you really like the style of and just tell them verbally.
