As a rule of thumb, the more abstract-costume-like an outfit looks, the less likely it is that it'll be sold to the public. These are mostly sold to the upper crust 1% who attend the shows, or only borrowed on request to VIPs for projects (music videos, photo shoots) or sometimes TV appearances. There are also couture lines who are only sold to mentioned very rich guests (Chanel couture, Armani privé etc).

And IF one of these special pieces indeed does get sold in stores, you need to be fast because they get soldout quick in boutiques.

Only very few of these items ever makes it to the public though. For example one of the last dresses Alexander McQueen personally made is being sold on eBay (or was, haven't checked on it for a while now).
