Well maybe that wasn't a great question because "fat" is so subjective. You can feel uncomfortable at a normal weight.

Well her eating healthily is good but skipping meals and only eating bites of your food isn't really a good sign imo and getting cold is a well-known sign of malnourishment.

You can still be insecure if you're in a relationship. It really has nothing to do with you or other people. Everyone could be complimenting her and she'd still feel the same way.

They don't have to look bad. You just need to find a good doc. Juvederm looks really good imo (pic related). I really want to try Juvederm someday. It's not permanent anyway so it's not that big of a deal if you don't love the way it looks.
