Anyone can wear it and it's certainly made in smaller sizes and for people with smaller pocket books but yeah I guess the point I'm trying to convey is that the guys who are holding up a lot of avant-garde designers bottom lines are buying stuff every season and wearing 48-52 or M-XL. If you watch web stores (or even visit physical stores) the smaller sizes tend to sell out quickly but that's because stores buy way less in those sizes rather than that they're the most popular. Just a misconception that is usually good to clear up.

Honestly most people would probably disagree with me on this but I feel there's a lot of leeway with off the rack stuff especially because it will most likely never fit anyone perfectly in the first place anyway. I pretty regularly size up 1 or 2 and don't have a problem. If it's something more fitted like a dress shirt I'd find one that fits you really well and then take it off and measure it all over, then compare those measurements with the ones of shirts you find online. I've generally found this is the best way to go about things when you're shopping online.
It's really easy for you to fuck up measuring yourself or maybe you do it slightly differently than how they do it in another country or whatever. I don't even think I know my own body measurements but I know my pit to pit, waist laid flat, inseam, rise etc. measurements off of all my favourite fitting clothes and then I just compare those to the measurements of what's being sold online. It's much more difficult for people to fuck up when they're measuring clothing you can lay flat on a table vs a human body, both parties included.

As for the shoe question look around, I see 45's pretty regularly and like I said they almost always make it to the deep sales.
