yeah when I was about 14 years old, I once volunteered for a catholic house renovation / building program for the poor and we were working on some trailer-house with metal paneling on the ceiling of the bathroom. It was really low ceiling so I thought it would be cool to rest my hand on it while I was taking a piss as I had never leaned on a ceiling while pissing before. Mutherfucking elecricity haunts my nightmares.

Pulling wells (repairing the shit half a mile under pumpjacks) and my leg kept giving out. Right side hurt. Whenever i touch the wrench to it i went down. For an hour i thought i was having a heat stroke or something. About every 5 minutes would try again and randomly bite it.

Turns out they threw the 440 line on the ground and rather than bury it asap just left it. My foot was in a puddle whenever i was getting hit. Piece of shit packrats chewed on the wire and started a fucking wildfire.

Silver lining tho. Firefighters saved 2 houses and us. Had i not been getting hit we never would have known until it was to late.

About ten years ago I worked in a government school infrastructure grants programme here in Australia. I don't know if it's still the case, but a single government owned company in a state here in Australia had the legal monopoly on all school maintenance, refurbishment, repairs, etc. There were actually two states like this, but I only visited one of them. They wouldn't do what you have there, but they wouldn't mask off shit either, so there'd be random paint strokes over outlets and switches, and paint all around the sides, paint dripped on carpet and desks. Just generally really shitty workmanship, and teachers/administration would complain to the government but nothing would get done. Same company operates in other states without that monopoly and their standards suddenly raise.

Ground fault. Hot wire connection came loose and is grounding on the metal yoke of the device. Yoke (and screw) is energized. The other screw is not making physical contact with the energized yoke and has a 0 potential. The meter picks up the 120v potential off the hot screw, compares it to the isolated screw (0 potential) and shows the 120v. If you touch that hot screw and anything else contuctive it gonna bite ya son.

This is a classic-seen almost identical situations. That meter is not being fooled. Handyman Hank came along and changed the switch himself, put in a new "3 way" which was actually a single pole switch. He used the green screw as one of the strap legs. Now it doesn't work right from both sides and the actual electrician comes in and fixes it. This of course doesn't blow the breaker on your standard plastic box.

I've done this before, it's actually an ingenious method of redundancy in case the switches themselves go bad. If that happens, I'm able to turn on the lights by simply holding a paperclip to both screws. So instead the head strips when you over-torque, so the machine is saved, but whatever you're screwing is now completely screwed. Yes the panel is plastic, the plan is to change it for a bigger one.
I’ll also connect the breakers in a non retarded way when I get the bigger box, and will add some extra brakers too for each new circuit that I’m doing.
No idea about how the hell fix the grounding though. It’s a second floor and almost impossible to bring the ground cable all the way down to “earth”. I mean, it’s doable, but I have to perforate two stories plus find a place to stick the grounding rod. I’m just paranoid while hammering down the rod I’ll perforate some huge water pipe that provides for the whole building. Any alternative ideas would be appreciated!
