I'm foreign-born living in the US and had this same experience with my ex.
My babymomma, however, is also a foreigner and we're both from coastal countries that eat a lot of seafood so we've never had this issue and we eat fish a few times per week.
Anyway, in my experience, Americans don't generally cook much seafood in general never mind whole fish. They "cook" some seafood in the form of pre-battered/breaded fishsticks or fillets or popcorn shrimp they reheat in an oven. Which is fine, I guess. My country has very little experience with beef brisket outside of soup and, in my region, stewing it with tomatoes but the most common way we eat it is from a can, encased in aspic while Americans corn it or braise it or barbecue/smoke it or stew it or pull it or numerous other things. They're just not that familiar with fish, which is no surprise considering the majority of this country is far away from any commercially fishable water.

In my experience, yes. With my ex, I recommended the silly cow eat the fish with bread so if any bones were missed, choking or throat irritation from tiny, hair-like bones would be less likely. That's how I ate fish as a kid before I figured out how to remove the bones from my portion.

We typically eat filets, but whole fried fish isn't especially uncommon. Bluegill and small panfish are typically scaled, gutted and fried whole with the skin on and eyes in.

It's not as uncommon as this thread makes it out to be, but whites overwhelmingly eat filets. I live on the Gulf Coast and have been a salt and freshwater fisherman since childhood. The only fish I can ever recall eating whole were bluegill, which was extremely common for fish frys, since you can fill two stringers full of them in a half hour with two pieces of bread. I can only recall seeing whole fish served in upscale restaurants, never in someone's home or at a party. I think it's more common in the north Atlantic region than it is in the south.

I've only had whole fish in two situations: a fancy restaurant, where they made a big deal out of deboning the fish for you at your table, and at rural buffets back when fish from the river were safe to eat (or were thought to be safe to eat). Once in a while, I'll see almost whole fish served but without the head. Guess the eyes really freak some people out. In general though, dealing with tiny bones is annoying so most people avoid it altogether because there are plenty of other options.

It's changing. Americans my age and younger are far more adventurous eaters than their parents or grandparents. One of my neighbours is a recently retired woman in her late 60s. She watches my son sometimes and she basically feels like family so we invite her over to lunch or dinner and even have her around for holidays occasionally.
