In America buffets are pretty much by definition unlimited, I've never heard of a buffet that isn't all one cares to eat.
In Japan, though, buffets can be all you care to eat but limited by time. Yakiniku or shabushabu tabehoudai is usually for an hour and a half or two hours total, but within that time go fuckin' nuts. Ditto at beer gardens.

waitresses may seat you at your table, sometimes, but always are the ones that bring beverages like a pot of tea and cups, sodas, ice tea from pitchers, or waters. They offer chopsticks and extra cutlery, extra napkins and clear used plates at every course. If they didn't immediately clearn plates, newbs would be tempted to eat off a plate and then return to the buffet to refill it with 2nds or items they meant to get but forgot. That's idiots for you. So sanitation absolutely depends on their being prompt and efficient. This is at every single buffet I've been to.
