For the first weeks/months output will be high and your body won't know to wake up if it fills up in the night. To save yourself a disaster, set an alarm half way through the night to empty it.

However, it's fuckin' hard to stay properly hydrated. Without a large bowel your body has a hard time absorbing water, and "just drink more" isn't an easy fix. You need to drink little and often. If you don't get used to hydrating properly, you'll be enjoying kidney stones in a few years.

You really need to cut out the carbs and snacks (which are entirely carbs). You're also likely not fully aware of how high your carb and sugar intake actually is, because that is ALOT of carbs and hidden simply sugars, and you seem to be under the impression that you aren't over-doing the carbs. I'm here to tell you that you are. Your diet is predominantly carbs, and all your problems stem from this. Do a clean keto diet for 2 months, and I guarantee that things will begin to clear up. Also look into fasting, you seem to be constantly feeding yourself with snacks if you aren't eating a meal and that is incredibly unhealthy and unnatural for our digestive system.

Pretty solid advice, BUT it's only one step of many in recovery. Let me add to what you said. Since most doctors don't give a hoot about you.

1.) Eating a GAPs or SCD diet is essential + probiotics
2.) You must ultimately address what's fucking up your system. It's often a parasitic infection, fungus, or pathogenic bacteria. Doctors have low success detection rates for all of these. So you might need to be your own advocate to get treatment. Unless your family has a history of gastrointestinal disease, the body doesn't often fuck itself, something is usually triggering it. Find that and you can heal yourself. Drugs you should research:

Mebendazole: hookworms, pinworms, whipworms, ascariasis
Praziquantel: tape worms, intestinal flukes, opisthorchis, clornorchis, schistsoma
Ivermectin: strongyloides
Triclabendazole: liver flukes

Those first three drugs should be required for all ibs and Crohn's patients. It would correct 70% of cases. The rest just need strong antibiotics and a poop transplant.

It's better than losing your intestines. Fuck this medical system and gastroenterologists who don't want you to be fixed. They want lifelong patients and to give you anti inflammatory medications and get mega bucks from surgeries.

That's a good thing honestly. I had the j-pouch and was just in the hospital because it ruptured and left a giant hole in my abdomen after the infection came to a head. I've also spent the past decade plus not being able to do much of anything outside of going to work and going back home because of needing to shit randomly with too much urgency too often to go out and do normal people things.
If I could go back in time I would have just asked for a permanent ostomy instead.
Have you tried one of the biologics yet (e.g. remicade / humira)? After a really long time with no meds I'm starting humira soon so I'll see if that helps maybe. Simpler explanation would be low blood sugar from malnutrition. Inflammation in the GI tract gets in the way of good nutrition even when you're eating and people with Crohn's tend to eat fewer calories because eating leads to painful traffic against ulcerated guts.
In the hospital when they put you on TPN (total parenteral nutrition; bypasses the intestines and gives you calories and nutrition through a picc line in your arm) they monitor your blood sugar every 4 hours since that sort of fundamental emergency nutrition source is so high in sugar.
After a couple weeks of going through that (i.e. ~84 separate blood sugar tests) it hit 112 at its highest and was usually below 100 the rest of the time.
I've also been below 130lb at 5'11 my entire adult life into my 30s now. You can get diabetes while being malnourished and sick from bowel problems but it's probably not the first explanation you should leap to when the other explanation of malnutrition and low blood sugar is a lot more straightforward an explanation.

I’m not exactly sure what I have but for the past decade, I have had strong to severe constipation.., I broke my l4l5 disc and never got it fixed, this is in the sacral nerve area which has nerves running to organs, I thought it was this but dr said no... but he’s work comp doctor so his job is to keep costs down for insurance company. I still have not loose stool. I also drink a whole gallon of water a day and my only diet is no breakfast or lunch and a big cup of yogurt for dinner with some non gluten kind oat mix, not very much but unsweetened yogurt is rough by its self... I will also eat a moderate sized amount of sunflower seeds... if I even try to eat something like a slice of pizza, a sandwich like pbj, fish, rice, etc... I get this strong bloat feeling and fatigue, I also get spasms in my guts in the past when I did this... very uncomfortable ones. I have had probably about 75 people cough in front of me in a odd way and about at least a dozen people hold their tummy showing discomfort while having a convo with me? This is very odd and something I never experienced before until this problem started... Probably not an odor of shit from being constipated, but you could have bacterial infection problems when you're not processing out food effectively and bacteria is what cause most of the noticeable body odor people get.
With the coughing thing my guess is you do have body odor going on but that it's not shit they're smelling from you being constipated so much as just stronger instances of the same body odor homeless people or the disabled get when they're neglected and haven't bathed in a while.
If you're not doing this already try switching to two long showers a day with plenty of high power soap. And if you can continue to look for ways to clear out your constipation problem e.g. (caffeinated) coffee.

The stories of people getting really fucked up and dying tend to involve patients who went against medical advice in favor of some alternative nutrition based "cure" nonsense. Treat it like you would any other niche trade where the person working for you probably has a pretty decent idea of what they're doing compared to yourself or other people not working in that field. Medical school takes a while for a reason.
You can look at people like Steve Jobs who had too much money and power and ditched mainstream medicine in favor of retarded dietary cancer "cures" as an example of why not to do that. They get hauled off to the regular hospital they were avoiding in the end, only by that point they let things get much worse and the doctors involved in treatment will have a lot fewer viable options with much lower success rates and much greater chances of severe complications.

Surgeons never do that (they care way too much about maintaining their track record for minimal deaths).
A long time family doctor on the other hand might go along with giving you enough morphine to kill yourself while in a hospital. It's *somewhat* legally condoned in fact (in the US anyway) under the doctrine of double effect where it's understood pain medicine needed to keep someone from suffering an inordinate amount and already dying from a terminal disease can have the secondary effect of also killing them, and it's not the same as assisted suicide if a patient dies as a consequence of pain medicine that was reasonably necessary for non-euthanasia / pain control purposes.
Even more likely than your family doctor would be a hospice. Hospices are designed for ceasing medical treatments that try to combat your terminal disease and focusing instead on keeping you as comfortable as possible while you die over the next few months or however long you have left.
About half of the US hospices out there will both know full well you want to be given assisted suicide with morphine and will be willing to do that for you (again under the guise of double effect to cover their asses).
All of them probably know this agenda exists, but the other half of them who don't do this might be too uptight about following the letter of the law or might overestimate their ability to provide you a comfortable existence no matter how sick and in pain you get before dying a natural death.
Anyway, your best bet is to not be an autist and not say on the nose shit like "can you help me commit suicide?" Treat it like a high school drug deal and don't give them reasons to be paranoid. Use euphemisms / read between the lines questions like "when the time comes can you make sure we do *whatever it takes* to treat my suffering?"

All an ostomy amounts to is a piece of your intestine cut off and pulled through your abdomen. Inside your abdomen your intestines are almost constantly moving bile, digestive enzymes, digested waste, etc. You just don't notice it until your colon fills up enough that you feel the need to take a shit.
With an ostomy instead the shit is just nearly always flowing out in much smaller amounts, kind of in a pulsing pattern. You don't ever voluntarily make shit come out of an ostomy stoma. It just drains into the bag until it gets full enough that you decide to dump the contents in a toilet.

It's not for convenience. It's because you have cancer or an autoimmune disease that ulcerated and tore apart your large intestines the point where they need to be taken out.
Some people just get a temporary ostomy at that point before switching over to having some spare small intestine looped around and attached to the anus to replace the large intestine, and for those people they would end up shitting voluntarily out the ass again.
But for others their disease would just keep recurring in whichever new length of intestine you used to replace the old one, so instead they do a permanent ostomy to allow shit to drain without having the inflammation associated with defecation. I had acute food poisoning in February 2019. A week or so after it happened I started to feel nausea, bloat and heartburn almost constantly. After I had my proctocolectomy and ileoanal anastamosis to replace it with looped around small intestine ("j-pouch," but I try not to call it that because it just confuses normies who think "pouch" means ostomy bag when it's actually an internal loop of small intestine) I found the only time I felt symptom free and able to eat whatever I want without needing to use the bathroom every two seconds was when I was on the percocets they gave me post-surgery.
But when I mentioned that the doctor didn't like the idea because it's "habit forming." Which yeah, no shit it'd be habit forming. It works, so you would keep taking it. But doctors are on edge about recreational opioid abuse, much more so today than back then even. And while I got more refills of it I stopped asking for it because I didn't feel like needing to argue with the doctor every few months to reup for something he clearly saw as temporary sooner or later (which makes no sense since it isn't a temporary problem it's fixing).
