It's called tragedy of the commons. Medicine costs $X to develop, $Y to manufacture, and needs to recoup $Z/year for the next e.g. 10 years for the pharmaceutical company to break even. Country #2 says "we will let you patent it here but you can only sell it here for $Y*1.01, if you try to sell it for more then we will simply make it here ourselves or buy it from another patent-breaking country who will sell it to us for $Y or less". Company grimaces and says fine you can buy it for $Y*1.01. Now it raises the price in the other countries because it needs to make $Z/year in order to break even on a reasonable time scale.
Countries #2-#200 pull the same stunt, cost goes up astronomically, eventually only country #1 is left as the sucker paying the price for every single leeching country out there.
