Figure out what carb source agrees best with your digestive system. For me it's potatoes or rice. Then add whatever you like to make it palatable - meat, soy sauce, sour cream. Don't overeat such that you're full for hours. Don't rely on ice cream/sweets, but mix them into your diet between meals, along with shakes and other snacks.

Serious answer: milkshakes. I used to work at a group home for men with brain damage. There was a guy who lost his normal sense of hunger and was dangerously underweight. The dietician told us to make sure he drank a milkshake every day on top of his normal meals and sure enough that did the trick.
