The bread was literally just baked minutes ago. This is called pannuozzo. It's a style of sandwich sold all over Naples and our surrounding areas. The bread is the same dough as used for pizza and baked the same way, taking mere seconds. Often, the bread is baked to order and sandwich constructed immediately after.

Just like any other style of sandwich, you get what you want. Rocket or other lettuces aren't uncommon nor are tomato or onion or whatever else. Roast pork and garlic spinach is my favourite.
