Portion sizes. Japanese restaurants serve portions roughly half of what you see in America. Wouldn't this be considered a serious disorder like hyperthyroidism? For most people though, they can choose the general diet they'd like to follow. They can choose roughly what their macros are and what foods they eat on a daily basis. They can estimate their TDEE as best possible, track their food and calorie intake as well as physical activity, weigh themselves weekly, track their progress, and see if it all matches up. If you should be losing a pound per week but aren't losing anything then you either messed up your calorie count or your TDEE doesn't match up with the estimate. They can make adjustments and try that for the next 4 weeks and see how it goes. Eventually you will figure out what your body needs to lose then maintain weight in a way that works for you. It really shouldn't be that hard though. I've done all different types of macros and foods, and it's never deviated from my estimated TDEE accounting for varying physical activity each day. More often than not people just don't track their intake properly.
