Can someone explain tipping a bit better to me.

I'm Australian and will be visiting America in a couple of weeks.

I've heard that it's customary to tip 15% of whatever your bill was to the waiter/waitress. Where did that number come from and why?

What I don't get is, why am I expected to tip more if I get the $25 meal off the menu as opposed to the $18 one when the waiter is doing the exact same job of bringing the plate out.

am I missing something?
