It was invented in Rome, by a Roman, and was popularized as Alfredo. It's referred to Fettuccine al Burro because Alfredo is that chefs take on the original folk dish. Al Burro of course meaning 'Butter'. Because that is what it is, Fettuccine and Butter, with some good cheese, and nothing else beyond maybe some cracked pepper.
