You will see different signs depending on the stage, since many early stage teams can get past one stage but not others. If we define early stage success (past attaining a state of failure) as $1MM in revenue (a clear sign of value) and we avoid the obvious failure of not having any product; I would say the top three are...

1. The founders want to make "easy money", either through the mere application of someone else's capital and/or by making too slight a derivation of an already market-tested product eg: 7 Minute Abs!. Both of these mean that they are confused about how "hard" startups are, entirely driven by extrinsic rewards and are therefore; in for a motivation sapping awakening.

2. The investors are impatient, don't understand venture investing and have control. Why? When they don't receive the return they are used to, they will get antsy and start making the wrong decisions to "save their investment".

3. The founders do not have a clear understanding/idea of their companies competitive landscape and where they fit in that landscape. Why? They are mesmerized by the value they want to provide without seeing the reality which their customer/the person who determines their success or failure, exists/decides in.

This last one is the most heart breaking, because many smart, intrinsically motivated and honorable people fail because they refuse to "seek the truth". Why? The reasons are varied, but it is mostly fear.
