Those countries don't have free healthcare, they have the equivalent of medicare but it is usually managed by subnational governments, like alberta or scotland, which leads to wildly different healthcare depending on how corrupt the particular province is. There is nothing stopping California from changing mediCAL to become like the NHS, but it would require even higher taxation of their declining tax base which is unfeasible due to badly mismanaging their society. In fact England's NHS and Ontario's healthcare system are both failing because of how badly managed they are.
