The server monkey who installed the storage system for the VM cluster fucked up the setup and forgot to configure the secondary controller at all. This went unnoticed for a length of time (roughly two years iirc?) until the primary controller puked its guts out at 4 AM, at which point the storage system manager switched over to the secondary controller as intended. However, since the secondary controller was still in its factory default configuration, it happily went ahead and initialized the disk array for first time setup, instantly nuking the entire VM cluster and bringing down EVERYTHING, including networking services. It was basically a paid day off for everyone except the IT department which was absolutely on fire, eventually the server guys got the environment restored from the tapes but the day was well and truly ruined at that point.

Wagie coworker accidentally mixed two different containers of clearcoat together because he thought one was the hardener. So when the clear was sprayed, it was unable to properly cure.

Caused about 30k in damage. Original 8k paint job was ruined, then everything needed 100 hours of cleanup and to be re-preped and then completely redone. Overall about 400 hours of shop labor lost and customer was beyond angry.

When I was a private in the army I had ammo duty at the range. Normally you keep track of the numbers of rounds used but I got sent to go shoot and when I got back like 4 hours later the replacements hadn't kept track of anything. I used to go smoke weed at lunch at one of my jobs where I would literally drug test people for new hire and random tests kek. Well this family friend came in and failed and lost his 6 fig job for weed I don’t know what or how it happened but I think I contaminated his urine somehow bcuz I tested him right after coming back from lunch. It was a lab pee test so I only did the collection process and I think particles fell into the cup. He got turbo fucked and still hasn’t recovered. Told nobody about that.

Then I had a friend who popped hot for meth on an instant pee test and I told him wtf dude but turned out zantac popped positive on those tests but I helped him out by swapping his sample for clean pee that I had saved in my car for myself (it had to go to a lab for verification of the meth positive)
Well, that clean pee sat in my glovebox for about a year and it disintegrated into a goo and the lab said wtf is this you did not pee in this cup this is just goo.

And then final story I got caught stealing my bosses dogs pain pills out of the bottle on the desk and got fired.

There were no testing simulators, got to test it one time on the field.
Right before final presentation randomly found out a bug that if power for the remote shuts off the last input keeps on looping.

If the wagie has been at the company for like 5 years then it is much harder. It is one of the ONLY good parts about being a long term wagie. Yeah it was always a mixed bag of who you got for your coworkers. Some were genuinely kind hearted people who were passionate about helping the youth then eventually got jaded(me), former foster youth and fuck ups who got their shit together, people who did/made drugs and occasionally fuck each other yet were somehow pretty good at the job, the jokester Mexicans who used to be gangbangers, those who tried to be military dads demanding respect and would yell at the kids pretty much getting them escalated, and the vets who are just really good at the job and are well liked.
I got lucky and worked with a group of guys that seemed like a bunch of class clowns yet were in the toughest house out of the agency. Work place politics were far away from us so we never had to deal with that bullshit, but at the same time it took forever for backup to come. I also worked with this one dude who revealed his power level and we would occasionally just say 4chan memes in front of others and the clients and they were none the wiser.

The owner of a company of 100ish people company bought out a smaller company that does similar work in the same industry but not exactly the same. The new owner pissed off everyone in the smaller company he bought out so every single one of them left within the span of about a year. Now the load of that company was transferred onto a few people from the company I worked for. Not knowing much about their products, or hardly anything about their customers and past projects inevitably made things things go bad. I'm pretty sure several projects got totally botched. The chaos and increased work load made several more people from the parent company quit. I would guess they probably lost pretty much all the value from the smaller company as well as a fair bit from the larger company.

Was a support for ecommerce ops for a large consumer products manufacturer that also did first party retail sales too through their own site and other channels. I have never in my life seen someone torch so many customers and create negative brand equity that easily did not make their salary worth what they were saving giving a 10 year lead 50 cent raises every year. She would demand they were to pay return shipping expenses. We were getting discounted rates already set up from a very large online retailer...

What would cost us $8-$15 for a large over sized return would easily cost people no less than $40-$50 retail shipping when they go to UPS/Fedex

If the customer really wanted to make things worse for us they would go to USPS who absolutely destroys you in expenses for large dimension items. Now it could be anywhere under $100 because of course they would send it USPS Priority

Time after time they would file chargebacks, blow up our communication lines, and of course, leave reviews detailing this part of the process

We would refund fully on the charge back and many times the inflated return cost. I'd be surprised if 3-5 customers didn't do this daily.

The hangar that houses the planes has a special fire suppresion system that mixes foam with the water, it helps put out jet fuel fires. Well, silver lining was that the pipes with the foam were so corroded from years of neglect that they burst from the pressure, so no foamy water in the hangar, just normal nasty rusty water. However, all 3 planes in that hangar had their windshields removed for maintenance, so the water got inside the cockpits and ruined every single device and all the avionics equipment. Nearly 3 million dollars in damage. Plus a week of cleaning and repairing the damage.

IT was never going to be that in the 21st century because of the ubiquity of high speed LANs and broadband Internet. Almost anything IT needs to do to a machine, even installing an OS on bare hardware for many system types, can be done with scripting and automation over the network. At that point it's cheaper to pay one guy 2-3x as much to get the work done 10x faster. Why would any business ignore free money like that? It's also not the sort of thing unionization would have prevented because they would be bulk fired and replaced by software rather than people.

Depending on the job, its also the level of burnout we faced on an emotional level. Its how the vicious cycle in the system came to be. Think about any teacher you had back before you were an adult. Think of any of the teachers who were pricks or constantly over everyones shit and stopped giving a fuck. At some point in their life they were someone who wanted to make a positive impact in a child's life. Nobody goes into teaching for the pay and many of the rewards you get are from the lives you change. I dropped out of a masters program for school counseling partially because I could see some of the pitfalls many could fall into and I know myself well enough to where Id probably give up after a few years. It truly is a sunk cost fallacy that many teachers go into by staying in the profession while being so miserable for so long. Doesnt help that standardized testing made this worse by 10 fold

I was working at a big pharma company a couple of years ago. We were working on a vaccine and thought it was safe and effective. Now unknown deaths are up 40%. Oops, oh well, my bonus was great so it all worked out.
