Do people actually even take advantage of equity?

My grandparents are in their late 80s, they bought their house for like $15k back in the 1950s, its worth $4 million now, they haven't touched any of their equity.

So either theyre doing something wrong by not accessing it, or equity is kind of useless

They're doing something wrong. If they were jewpilled they would have taken out a loan against the equity when rates were near zero and then invested the money from the loan.
