Okay, here's the deal. If they're asking you for your money to invest in your sister's clinic, you have to realize you're not just "doing them a family solid". They're probably expecting to either not have to pay it back, or pay it back interest free. The fact that they're intending to use your capital to start a business means that you should treat it as venture capital and assume that you want to see a return on your investment.

Ask them for information. Assess their risk like you're investing in your sister. Ask her where she's planning on renting. Ask her if she's hiring or doing it herself. Ask if she plans on staying open during corona digging around in peoples mouths, otherwise shes wasting your time.

Do up a contract if they want your money. Tell them you want to see either a percentage return by a certain date on your money, because you were planning on investing it in other stuff, and you shouldnt have to make sacrifices just because your family feels entitled to your startup cash.

If they're not willing to treat this like an ordinary loan and want special family deals to mitigate their risk, then you should have red flags about her business and its viability. GET A CONTRACT, if you do this under the table you won't see a lick of money out of it (because if they're willing to treat you like an asshole for not wanting to lend it, they're going to treat you like an asshole for trying to collect it) and you will have no legal recourse.
