A good 90% of the world didn't know about proper hygiene and infection control techniques though, manners that get taught in grade school to us (e.g. cover your mouth when you sneeze/cough, wash your hands all the fucking time etc.) were a foreign concept back then. Even if people now don't practice proper hygiene, they'll be shunned in the western world by people who do. If you've ever been annoyed by potential consequences of someone coughing near you, then you know more than most people did in 1918.
