There was already a drug that does this, called Glybera. And it's ultra-expensive shit that is no longer made. A much smarter option is statins which lower LDL and decrease the risk of atherosclerotic events. And omega-3 fatty acids to reduce triglycerides.

They'd rather drugs that create permanent dependencies and are expensive enough that they can make the equivalent profit that OP envisions in curing the world just from a smaller number of patients in the first world.
And that's just for unequivocal diseases - they manufacture subjective illnesses like depression or even create positive "identities" requiring pharmaceuticals like transgenderism, and in those cases they're worse than vultures.

I know because I looked into releasing a pet pharmaceutical, which need to go through FDA trials to be marketed legally.

It can cost as low as 80k to go through trials, which you get back for some drugs. Seeing as this will be a human pharm, it will probably cost more - thankfully Trump passed legislation making it easier for new drug makers to enter the market - so whatever the cost - it comes with the rebate where you get your money back.

Consider.
You would have to raise capital to show clinical trials because these people won't believe you unless you got proof. What I recommend is that you go to one of competitors in the space , and sell your idea to them. (Or contact their Venture capital arm such as Pfizer corporate venture) Since this isn't a small project considering supply chain and logistics and all of the sourcing of ingredients , it would take time and consider human trials which would take years plus regulations. You should sell it and make a fuckload and move on to the next thing. I'm sure your competitors would be interested in what you have to offer. They got money and you got the cure. What do you want more? You

How can you be sure that increasing lipase production in macrophages/WBC doesn't have an adverse effect - i assume that it will be all macrophages you will change the gene expression profile off and not just those associated with the plaques.
