The last one I worked for (hardware startup) had plenty of issues and I wrote them down to not repeat those mistakes (or at least mitigate them) when I do my own stuff.
But it boils down to:

* stupid opportunism from the leadership (jumping on every new trend, immediately forgetting about what was considered to be hot just yesterday), changing much of what's important every quarter;
* no guiding principles (see above), so no understanding of where one want to go or why or how;
* no desire to understand the wider industry, its problems, circumstances of doing business, best practice (and when to use them or deviate from them, but it's hard to break rules when one is not even interested in knowing those rules);
* very bad internal communication, that's the main killer next to the stupid opportunism: no or little updates, only selected information instead of providing the big/whole picture which would give engineers a better understanding of what's needed, no real feedback to the decision makers, top-down command structure based on hype and lack of understanding;
* pretty bad external communication: while the self-promotion / marketing was rather okay, the communication was dishonest and with potential or actual customers very incomplete and often based on neglect and outright hate for the customers' questions and wishes, both the sales and the customer service processes were accordingly bad and many (potential) customers moved on.

That all amounts to jumping from one hype to the next, using empty phrases, quickly recruiting people into important hype positions (to look good to potential investors) with power over knowledgeable employees, neglect of customers and engineers, pragmatic lies to sponsors and investors and the general public.
