In practice, the Fed uses the repo market to provide liquidity to banks, which then becomes shadow money as an interbank loan to smaller banks. If enough smaller banks crash, then the big banks become insolvent and collapse, bringing most of the financial system down with it. This could easily happen with a black swan event like coronavirus, or oil dropping 30%, or any major geopolitical intervention in the countries that owe that interbank debt.
