It seems your application requires a bit more information than mine did. I didn't have a separate form to fill out either. There was a section on residence/living arrangements where I said I was living at X address with parent paying $75 a week.

I've just checked my statement and pic related is what I get paid. There's no mention of rent assistance or anything like that (which is a separate payment). So perhaps my living conditions allowed me to receive the maximum youth allowance possible?
